What is Support Coordination
Support Coordination is an NDIS-funded service that helps you understand and implement your NDIS plan. Our Support Coordinators work closely with you to:
Understand your NDIS plan and the options available to you
Connect you with the best service providers to meet your needs
Negotiate services and prices to get you the most effective support
Help you budget your NDIS funds
Assist you in engaging with your local community
Arrange necessary assessments and reports
Develop service agreements with providers
Monitor your progress and communicate with the NDIS
Prepare for plan reviews and set new goals
Our Support coordination levels to suit your needs
We know everyone’s needs are different. That’s why we offer three levels of Support Coordination:
Support Connection
If you just need a little help here and there to connect with services and understand your plan, this is for you. Your Support Coordinator will give you the tools to manage your supports on your own.
Support Coordination
If you need more regular help for your plan, this level is perfect. Your Support Coordinator will work with you to figure out your needs, connect you with services, and help you work towards your goals.
Specialist Support Coordination
If you have more complex needs this high-level support is here for you. Your Support Coordinator has skills to help you navigate challenges and get the support you need.

Are you eligible for Support Coordination?
